As a new patient at Robbins Dermatology, we look forward to providing you with outstanding dermatological care. Please read the following information so you know what to expect during your first visit with us.
New patients will need to fill out a check-in sheet and a patient history form, available online or at the clinic the day of your visit.
Our office may be contacted to schedule appointments at (307) 672-8941 during office hours — after hours, you may leave a message for us to call you back— or you can request an appointment via the online form below.
Insurance: Insurance is a contract between you and your insurance provider, covering portions of your care based on your plan. While we submit claims as a courtesy, please be aware of your coverage, co-pays, and deductibles, and be prepared to pay your portion at the time of service.
Patient Forms
New Patient Forms: for your convenience, print out and complete the following forms before your visit to our clinic, and remember to bring your driver's license and a copy of your current insurance card.
Privacy Practices: a copy of our privacy practices can be found below: to comply with HIPAA regulations, we must also have a signed receipt indicating that you have received a copy of these privacy practices.
Authorization for the Release of Medical Records: you may print this form and fax it to your previous physician in order to have your records sent to Robbins Dermatology.

Appointment Request: During business hours, the best way to make an appointment is to call us at (307) 672-8941. You can also request an appointment through our online form if you prefer. Please fill out the form, click submit, and we will contact you on the next business day regarding an appointment.
